The role is based in our Hospice Center, where we prioritize care and compassion for both patients and their families. Although this position is remote, you will collaborate closely with a multidisciplinary team across various locations. Key Responsibilities\n

• Conduct thorough evaluations and assessments of children under early intervention programs, utilizing evidence-based practices.

• Participate proactively in the development and implementation of Individualized Family Service Plans (IFSPs).
\n
• Monitor and document outcomes as a vital member of a collaborative team, ensuring adherence to compliance regulations.
\n
• Provide tailored support and resources to families, empowering them to enhance their child's development through informed strategies.

• Develop rapport with children, families, and team members, fostering a nurturing and effective learning environment.
\n
• Implement the IFSP for enrolled families, adapting services according to the unique developmental needs of each child and family dynamics.

• Employ coaching models and natural environment teaching techniques to facilitate optimal child development.
\n
• Maintain high standards of health and safety for all individuals served, complying with all regulatory protocols.

Patient Population / Client Focus\n

You will primarily work with children experiencing disabilities or developmental delays, ensuring they receive the support needed in their formative years. This role involves working closely with families to promote skills in physical, cognitive, social/emotional, and communication areas. The focus is always on providing compassionate care and practical solutions that empower families and enhance children's life skills.

Qualifications\n

• Master's Degree or Doctorate in Social Work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) by the Department of Public Health.
