This full-time, onsite role offers a unique opportunity to work closely with experienced healthcare professionals in an outpatient setting, specifically within our Cardiology Clinic. As a vital member of our multidisciplinary team, the NP will play a crucial role in enhancing patient care by working collaboratively under the guidance of the Chief Nurse Practitioner and the CACC Medical Director.

Key Responsibilities

• Conduct thorough patient assessments by obtaining detailed medical histories and performing both comprehensive and focused physical examinations.
• Order and interpret diagnostic tests, manage chronic conditions, and address acute medical issues within the scope of practice.
• Develop individualized, goal-oriented care plans that include health maintenance and appropriate interventions in alignment with established protocols.
• Engage with patients and their caregivers in counseling sessions to educate them about disease management and treatment options, ensuring they feel informed and supported.
• Collaborate with physicians and other healthcare professionals to adjust treatment plans as necessary based on patient progress and feedback.
• Coordinate patient care by facilitating hospital admissions and discharges, working closely with the inpatient care team for seamless transitions.
• Document all patient interactions in the electronic medical record system, ensuring accurate and timely record-keeping.
• Participate in research initiatives and utilize findings to enhance clinical practice and patient care strategies.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve adult patients, including those with complex cardiovascular conditions, such as advanced heart failure, pulmonary arterial hypertension, and those recovering from heart transplant surgery. Additionally, the NP will be involved with patients requiring advanced therapies such as intravenous and inhaled medication support, fostering a comprehensive understanding of their unique healthcare needs.

Qualifications

• A Master’s degree in Nursing, specifically as a Nurse Practitioner with a focus on adult care.
• Current New York State Registered Professional Nurse (RN) and Nurse Practitioner licenses.
• NP Board certification, ensuring a recognized standard of clinical knowledge.
• Current certifications in Basic Life Support (BCLS), Advanced Cardiac Life Support (ACLS), and a valid DEA registration.
• Proficiency in electronic medical records and standard office software including MS Word, PowerPoint, and Excel.
• Ability to communicate complex medical information clearly to patients and their families.

Preferred Experience

• At least two years of nursing experience in a hospital setting, particularly within a cardiology department.
• Experience managing patients with advanced cardiac conditions and familiarity with outpatient care protocols.
• Knowledge of coordinated care approaches, working within a multidisciplinary team.

Compensation and Benefits

The position offers a competitive salary ranging from $120,000 to $220,000 per year, commensurate with experience and qualifications.
