Work Environment

This position will be based onsite at our assisted living facility in Kingston, New York. Key Responsibilities

• Conduct extensive health assessments and screenings to evaluate the cardiovascular health of residents and ensure preventative measures are in place.
• Diagnose and manage both acute and chronic medical conditions affecting cardiac patients, including establishing treatment plans and managing medication regimens.
• Engage in preventative care efforts by offering education on lifestyle changes, nutrition, and exercise tailored to cardiac patient needs.
• Collaborate with community organizations, healthcare practitioners, and family members to enhance the social, emotional, and physical health of residents.
• Execute detailed and accurate documentation of healthcare records in compliance with healthcare regulations and best practices, ensuring confidentiality and proper record-keeping.
• Actively participate in community outreach initiatives, health fairs, and awareness campaigns to promote cardiac health and provide free health screenings.
• Keep abreast of the latest evidence-based practices in cardiology and community health trends, integrating new learning into patient care approaches.

Patient Population / Client Focus

Our patient population primarily consists of residents in the assisted living facility, many of whom may have chronic cardiac conditions, mobility limitations, or other health concerns due to age. The Nurse Practitioner will focus on delivering culturally sensitive care tailored to the unique demographics of our residents, promoting health equity and advocating for patients' rights while navigating the healthcare system.

Qualifications

• Current Nurse Practitioner credential and active State Nursing Licensure in New York.
• A Master's degree in nursing from an accredited institution.
• A minimum of two (2) years of clinical experience in a cardiology setting, with a strong understanding of cardiovascular health.
• Superb interpersonal and communication skills, enabling effective interaction with patients, families, and interdisciplinary teams.
• A proactive commitment to social justice and health equity, with an emphasis on advocating for vulnerable and underserved communities.

Preferred Experience

• Experience working with elderly patients in managed care or assisted living environments.
• Additionally, familiarity with electronic health record systems to streamline documentation and patient tracking processes.
• Ability to lead community health initiatives and educational programs effectively.

Compensation and Benefits

The compensation for this full-time position ranges from USD 150,000 to 160,000 annually, depending on qualifications and experience.
