This role will be conducted remotely, providing the flexibility to manage your work from a location that suits you best. However, the work will be deeply aligned with our Hospice Center's operations, emphasizing collaborative care and a commitment to providing high-quality support to young children with disabilities and their families. Key Responsibilities\n

As a Licensed Clinical Social Worker at Foresight Psychotherapy, you will be responsible for a variety of essential functions, including:

• Conducting thorough evaluations and assessments of children to establish individual needs and develop targeted support plans.
\n
• Participating in the development and implementation of Individualized Family Service Plans (IFSPs), collaborating seamlessly with team members and family members to ensure comprehensive care.
\n
• Monitoring and documenting outcomes as a key member of the treatment team, ensuring compliance with all federal and state regulations governing practice.
\n
• Providing educational support to families, sharing critical information, skills, and resources to enhance child development and overall family well-being.
\n
• Utilizing evidence-based practices and the Natural Learning Environment model while conducting sessions in a manner adaptable to each child's unique learning style.
\n
• Developing and maintaining positive rapport with children and their families, fostering a supportive and nurturing environment for growth and development.
\n
• Administering evaluations and assessments using standardized tools and maintaining accurate records in compliance with organizational standards.

Patient Population / Client Focus\n

Your primary focus will be on children and families, especially those experiencing delays or disabilities. You will work closely with infants and toddlers, helping them learn crucial developmental skills across various domains such as physical, cognitive, emotional, and communication development. Your role is critical in advocating for these children, creating customized strategies that align with each family's goals and priorities, ultimately striving for successful and fulfilling outcomes.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work is required.

• Must hold a valid LCSW license issued by the Department of Public Health.
\n
• A valid driver's license is necessary for any potential onsite visits or community outreach engagements.

• A minimum of one year of experience working with children in early intervention is required.

• Commitment to the Natural Learning Environment Practices is crucial for success in this role.

Compensation and Benefits\n
