Full-Time Physical Therapist for Home Care Services

Position Overview

Focus Physical Therapy is seeking a dedicated and experienced Physical Therapist to join our compassionate team in Port Saint Lucie, Florida. In this full-time role, you will be responsible for providing exceptional patient care in a home care setting, diligently implementing individualized rehabilitation plans tailored to meet the unique needs of each patient.

Work Environment

This position is onsite, allowing you to work closely with patients in their home environments. Key Responsibilities

• Conduct comprehensive assessments of patients' physical abilities and rehabilitation needs to develop customized treatment plans aimed at enhancing mobility and quality of life.
• Guide patients through tailored therapeutic exercises and modalities, including electrical stimulation and manual therapy, to facilitate healing and pain management.
• Educate patients and their families on home exercise programs and strategies to maintain progress outside of therapy sessions, ensuring effective recovery.
• Monitor and document patient progress meticulously, including regular evaluations of range of motion and strength improvements to inform ongoing treatment adjustments.
• Maintain a clean and organized treatment environment, ensuring all equipment and supplies are readily available and functioning optimally for patient care.
• Collaborate with interdisciplinary teams, including Occupational and Speech therapists, to ensure comprehensive patient care and integrated treatment approaches.
• Provide emotional support and motivation to patients, promoting a positive atmosphere that encourages persistence and healing throughout their rehabilitation journey.
• Adhere to established ethical, professional standards and practices, ensuring patient confidentiality and respect is upheld at all times.

Patient Population / Client Focus

In this role, you will work with a diverse patient population, including individuals recovering from orthopedic surgeries, neurological impairments, sports injuries, and age-related mobility challenges. Your expertise will play a critical role in their emotional and physical recovery, providing them with the tools to regain their independence and enhance daily living.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• A valid physical therapy license in the state of Florida.
• Demonstrated ability to develop and implement effective treatment plans based on specific patient needs.
• Strong communication skills to effectively educate and motivate patients and their families.
• Ability to assess patient conditions and make timely adjustments to care plans as needed.

Preferred Experience

• A minimum of one year of experience in a physical therapy role is required, with a preference for those with home care experience.
• Familiarity with electronic medical records (EMR) and documentation standards in healthcare settings.
• Experience working collaboratively within a multidisciplinary team.

Compensation and Benefits
