Work Environment

This position is primarily onsite, as you will be traveling to patients' homes within Port Saint Lucie and its surrounding areas. Key Responsibilities

• Conduct comprehensive assessments of patients to determine their physical limitations and healthcare needs, followed by the development of personalized treatment plans aimed at their rehabilitation goals.
• Utilize various evidence-based interventions and modalities, including manual therapy techniques, therapeutic exercises, and modalities such as ultrasound and electrical stimulation, to enhance functional outcomes.
• Regularly monitor and document patient progress in a detailed and organized manner, ensuring that all changes in their condition are accurately recorded and communicated.
• Work collaboratively with physicians, occupational therapists, and other healthcare professionals to coordinate treatment plans and optimize patient care.
• Educate patients and their families on exercises, self-management techniques, and other strategies to assist in recovery and pain relief, promoting long-term self-sufficiency.

Patient Population / Client Focus

Your primary focus will be on patients with varying physical limitations, disabilities, and pain issues, including post-surgery recovery, chronic pain management, and elderly care. By delivering tailored interventions, you will significantly impact their recovery and functional independence, enhancing their overall quality of life.

Qualifications

• Active licensure as a Physical Therapist in the State of Florida.
• A Bachelor's Degree in Physical Therapy from an accredited institution. Advanced degrees such as a Master's or Doctorate are welcomed.
• Strong knowledge of human anatomy, physical therapy principles, and treatment methods.
• Excellent communication, interpersonal, and organizational skills, essential for motivating patients and working effectively within a healthcare team.
• Demonstrated ability to manage a dynamic caseload and maintain productivity without compromising quality care.

Preferred Experience

• Previous experience in home care or outpatient settings is highly desirable.
• Experience with specific populations such as geriatrics, post-operative care, or neurological conditions will be considered an asset.
• Proficient in electronic medical records and documentation practices preferred.
