Key Responsibilities

• Conduct thorough screenings and assessments of patients to evaluate their physical capabilities and limitations.

• Develop personalized treatment plans based on patient evaluations, incorporating evidence-based practices that align with standard physical therapy modalities.

• Consult with and educate patients and their caregivers regarding treatment processes, home exercise programs, and overall health maintenance.

• Utilize proper techniques to deliver physical therapy treatments, ensuring safety and effectiveness.

• Document patient progress and treatment outcomes promptly and accurately in accordance with facility and regulatory requirements.

• Maintain regular communication with patients, families, and other healthcare professionals to promote a coordinated approach to patient care.

• Stay current with advancements in physical therapy practices and engage in ongoing professional development opportunities.

• Participate in quality improvement initiatives and assist in operational oversight, including mentoring new staff and managing resources effectively.

Patient Population / Client Focus

Your role will primarily focus on working with a diverse range of patients in their homes, including individuals recovering from surgery, those with chronic conditions, and pediatric patients. You will leverage your clinical expertise to provide tailored therapeutic interventions that cater to their specific rehabilitation needs.

Qualifications

• Current State Physical Therapist license is required.

• A Master's or Doctorate in Physical Therapy from an accredited program is essential.

• Demonstrated experience in a clinical healthcare setting is preferred.

• Ability to communicate effectively with patients of all ages and their families, as well as with interdisciplinary healthcare teams.

• Physical capabilities to perform essential job functions, including lifting, standing, and mobility.

Preferred Experience

• Experience working with pediatric patients is a plus, as is familiarity with home care settings.

• Strong knowledge of rehabilitation practices, rehabilitation equipment usage, and home exercise prescriptions.

• Proficiency in electronic medical record (EMR) systems or similar documentation software.

Compensation and Benefits

This full-time position offers a competitive salary ranging from USD 50,000 to USD 75,000 annually, commensurate with experience.
