This position offers a fully remote work setting, enabling you to support clients from the comfort of your own home while maintaining a strong connection to our team. Typical work hours are Monday to Friday from 9:00 AM to 5:00 PM.

Key Responsibilities\n

• Act as a key liaison between clients, their families, and outpatient providers, ensuring seamless communication and collaboration for optimal care.

• Engage with insurance companies to manage pre-certification and utilization management, enabling clients to receive appropriate services and support.
\n
• Participate actively in discharge planning and aftercare planning, tailoring care strategies that meet each client's specific needs.
\n
• Provide therapeutic meal support by modeling healthy eating behaviors, helping clients to cultivate a positive and affirmative relationship with food.
\n
• Deliver compassionate care that is responsive to the unique circumstances of each client, including attention to their intersecting identities.

Patient Population / Client Focus\n

Your work will center around a diverse range of clients, particularly focusing on older adults and individuals facing life-limiting conditions. As a Clinical Social Worker, you will engage with clients experiencing complex emotional responses associated with grief, loss, and health transitions. Your expertise will help to improve their overall quality of life and will play a crucial role in supporting families during challenging times.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is a prerequisite for this role.

Compensation and Benefits\n
