Breakthrough Rehabilitation & Treatment Center is actively seeking dedicated and skilled Senior Physical Therapists to join our dynamic team in Old Bridge, New Jersey. Work Environment

You will be working onsite in our modern outpatient office, equipped with the latest therapeutic tools and technology to support your practice. Our facility promotes a collaborative atmosphere where interdisciplinary communication is valued, encouraging you to work closely with other healthcare professionals to develop comprehensive treatment plans. With regular hours from 8:00 AM to 5:00 PM, Monday through Friday, our schedule is designed to support work-life balance while maximizing patient care.

Key Responsibilities

• Conduct thorough assessments to evaluate patient needs based on their medical history and specific functional limitations.
• Create and implement personalized treatment plans tailored to the needs of each patient, ensuring adherence to state practice acts.
• Perform detailed evaluations incorporating standardized assessments and clinical observations, documenting findings accurately to inform ongoing care.
• Adjust treatment based on patient progress and feedback, recognizing shifts in patient conditions that require modifications to the intervention plan.
• Provide clinical consultation to healthcare team members and attend facility meetings to discuss patient progress and care strategies.
• Assist in setting measurable goals for patient recovery and clearly document each step of the patient's journey, including initial evaluations and discharge summaries.
• Complete mandatory corporate training and maintain compliance with all local and federal healthcare regulations to ensure high standards of practice.

Patient Population / Client Focus

Our facility serves a diverse patient population requiring physical therapy services for a range of orthopedic conditions, post-operative recovery, and rehabilitation from injuries. You will engage with patients of all ages, emphasizing individualized care that respects their unique challenges and encourages their active participation in the healing process.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited institution.
• Current licensure as a Physical Therapist through the New Jersey Physical Therapy Examining Committee.
• A minimum of two years of experience in outpatient orthopedic physical therapy settings.
• Current certification in CPR to ensure patient safety in all therapeutic settings.

Preferred Experience

Experience with advanced physical therapy techniques and modalities, familiarity with electronic health record systems, and a commitment to lifelong learning through continuing education are preferred. Experience in a collaborative healthcare setting will be advantageous.

Compensation and Benefits

This position offers a competitive salary in the range of USD 100,000 to 120,000 annually, accompanied by a comprehensive benefits package.
