Key Responsibilities

• Conduct detailed psychosocial assessments to formulate individualized treatment plans based on established patient needs and goals.
• Engage with clients in therapeutic sessions, using evidence-based practices such as solution-focused therapy, behavioral activation, and motivational interviewing.
• Analyze patient progress and adjust treatment strategies as necessary while maintaining ongoing communication with other members of the care team.
• Facilitate interdisciplinary collaboration by actively participating in case discussions with doctors and psychologists to create a comprehensive care strategy.
• Monitor and document patient responses to therapy and medication adherence, emphasizing treatment efficacy and identifiable side effects.
• Utilize telehealth platforms to ensure continuity of care for patients who may require remote assistance.

Qualifications

Compensation and Benefits
