Work Environment

This role is primarily onsite, focusing on home care, which allows you to engage with patients in their familiar surroundings. Key Responsibilities

• Perform thorough evaluations, reviewing pertinent medical history and conducting specific assessments to accurately determine patient needs and establish treatment plans.
• Effectively communicate evaluation findings and treatment strategies to patients and their families, involving them in goal-setting to foster collaborative care.
• Implement evidence-based treatment protocols tailored to each patient's condition, utilizing individual assessment findings, and adapting to changes in their health status.
• Collaborate with multidisciplinary healthcare professionals to provide comprehensive care, recognizing when consultation is needed and documenting changes in patient conditions.
• Plan for patient discharge and follow-up care while ensuring appropriate documentation, including daily encounter notes and progress updates, to provide clarity on the patient's treatment journey.
• Demonstrate effective time management skills to meet productivity expectations, keeping an organized schedule and attending meetings punctually.
• Participate in community outreach and marketing initiatives that promote our services, contributing to building a positive reputation in the healthcare landscape.
• Engage in continuous education and professional development to enhance personal competency and contribute to the organization's growth.

Patient Population / Client Focus

Your primary focus will be on patients receiving home care therapy, which includes individuals recovering from injuries, chronic conditions, and surgeries. This role demands empathy and understanding, as you will work with a diverse population, including elderly clients and those with mobility challenges, striving to improve their independence and overall wellness.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.
• A High School Diploma or GED; licensing requirements as determined by state regulations for practice.
• Certification in Basic Life Support (BLS) for Healthcare Providers from an accredited organization.
• Excellent communication and interpersonal skills to effectively interact with patients and their families.

Preferred Experience

• While prior physical therapy experience is not required, familiarity with home care practices will be viewed favorably.
• Strong clinical skills with a commitment to evidence-based practice.

Compensation and Benefits
