Work Environment

Working onsite, you will primarily deliver therapy services in patients' homes, allowing you to engage with individuals in their natural living environments. Key Responsibilities

• Conduct comprehensive evaluations, including a detailed review of medical history and physical assessments to determine individualized treatment recommendations.
• Provide clear explanations of evaluation findings and treatment plans to patients and their families, ensuring they are integral in setting achievable goals.
• Implement evidence-based treatment protocols tailored to each patient's unique presentation and health care dynamics, demonstrating proficiency in a variety of therapeutic techniques.
• Work collaboratively with other healthcare professionals as necessary, continually monitoring and documenting any changes in patients' conditions to adapt treatment plans accordingly.
• Develop and execute effective discharge planning and follow-up care, ensuring thorough documentation of care activities, which includes initial evaluations, progress notes, and discharge summaries.

Patient Population / Client Focus

Our patient population consists of diverse individuals ranging from pediatric to geriatric, each with unique rehabilitation needs. You will encounter a wide array of conditions including orthopedic injuries, neurological disorders, and chronic pain, enabling you to apply a variety of therapeutic techniques to cater to disparate recovery pathways. Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• Current licensure to practice as a physical therapist in the state of Florida.
• American Heart Association BLS for Healthcare Providers CPR certification is mandatory.
• Strong communication skills to effectively explain treatment plans and educate patients and families.
• A commitment to professional development, continuous education, and active participation in team-oriented initiatives.

Preferred Experience
