Key Responsibilities

• Conduct thorough evaluations of patients, reviewing pertinent medical histories, performing specific assessments, and documenting findings clearly and accurately.
• Communicate evaluation results and treatment plans effectively to patients and their families, incorporating their input into goal-setting processes.
• Develop and implement personalized treatment regimens based on individual assessments, patient responsiveness, and current health care standards, ensuring continuity and quality of care.
• Collaborate with other healthcare professionals as necessary, documenting any changes in patient conditions and adjusting treatment plans accordingly.
• Manage discharge planning diligently, ensuring appropriate follow-up care and timely documentation of all patient interactions, including intake evaluations, daily notes, interim summaries, and discharge plans.
• Exhibit excellent time management skills by maintaining an organized schedule, participating in departmental meetings, and meeting productivity expectations.
• Engage in community outreach and marketing activities to promote Patient Plus Therapy and its services.
• Contribute to departmental goals, compliance with regulatory standards, and a culture of continuous improvement.
• Actively pursue ongoing education opportunities to enhance clinical skills and serve as a mentor for students and peers in the organization.
• Facilitate a positive patient experience, showing flexibility and adaptability in meeting diverse patient needs.

Patient Population / Client Focus

The primary focus of this role is on orthopedic rehabilitation, catering to a wide range of patients recovering from various conditions. This includes post-surgical patients, those with sports-related injuries, and individuals with chronic musculoskeletal pain. Our aim is to restore mobility, enhance strength, and improve the overall quality of life for each patient we serve.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited institution.
• Current state licensure as a Doctor of Physical Therapy (DPT) by the Physical Therapy Examining Committee.
• A minimum of two (2) years of clinical experience in an orthopedic setting.
• A current CPR certification is required to ensure safety in emergency situations.

Preferred Experience

• Experience with specific orthopedic rehabilitation techniques and modalities is highly desirable.
• Familiarity with electronic medical record (EMR) systems for documentation and patient management.
• Prior involvement in community outreach or marketing related to physical therapy services.
