Key Responsibilities\n

The Primary responsibilities of our Mental Health Therapist include:

• Conducting comprehensive psychosocial assessments to evaluate client needs and develop individualized treatment plans.

• Providing evidence-based therapy to address a spectrum of mental, emotional, and behavioral disorders. This includes adjusting treatment plans as necessary to reflect changing patient conditions.

• Collaboratively working within a multidisciplinary team to coordinate care and develop effective discharge plans for clients needing ongoing support.

• Acting as a liaison with external resources, including community services and additional mental health providers, to streamline patient care and enhance treatment effectiveness.

• Maintaining compliance with federal, state, and local regulations, and adhering to the standards set by the Mental Health Practice Act.

• Managing daily priorities by assessing the needs of new, current, and discharging patients, ensuring timely and appropriate care delivery.

• Conducting supportive consultations and providing crisis interventions when necessary, including bereavement support.

• Engaging in effective communication, both with clients and external agencies, to navigate complex psychosocial factors impacting patient care.

• Identifying patients at risk for complications such as avoidable delays or readmissions, and coordinating interventions for optimal patient outcomes.

• Providing support and debriefing for colleagues when responding to high-stress situations or adverse events, enhancing team resilience.

Qualifications\n
