Work Environment

This position is designed for professionals who thrive in a hands-on, dynamic work setting. Key Responsibilities

• Conduct comprehensive evaluations of patients in their homes, assessing physical function, mobility limitations, and pain levels.
• Develop and implement individualized treatment plans utilizing a range of therapeutic modalities, such as therapeutic exercises, manual therapy techniques, and patient education.
• Monitor and document patient progress meticulously, making timely adjustments to treatment strategies based on observed outcomes and evolving patient needs.
• Educate patients, caregivers, and family members on home exercises, assistive devices, and adaptations to facilitate effective self-care.
• Collaborate with interdisciplinary healthcare teams, providing insights and recommendations to optimize patient-centered care.
• Engage in ongoing professional development through attending workshops, training sessions, and maintaining currency in best practices in physical therapy.

Patient Population / Client Focus

You will be working with a wide range of patients, ranging from children to elderly individuals recovering from surgeries, injuries, or chronic conditions. Whether it's helping a post-operative patient regain strength or guiding a child through rehabilitation from an injury, your role will directly impact their mobility, function, and quality of life. Providing care that respects each individual's unique circumstances and needs is paramount.

Qualifications

• Bachelor's Degree in Physical Therapy or higher (DPT preferred).
• Current and valid Physical Therapist state license.
• Excellent written and verbal communication skills to effectively interact with patients and their families.
• Proficiency in basic computer applications and electronic medical records systems.
• A strong commitment to patient-centered care and team collaboration.
• Adaptability and willingness to undertake a variety of clinical challenges.

Preferred Experience

• A minimum of two years of clinical experience in physical therapy is required.

Compensation and Benefits
