Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate patients' emotional, social, and environmental factors impacting their care, utilizing interviews, observations, and medical records.

• Provide counseling and emotional support to patients and their families, creating a safe space for discussing adjustment issues, grief, and coping strategies.

You will be serving a diverse patient population in hospice care, focusing on individuals facing life-limiting illnesses. Qualifications\n

• Master's Degree in Social Work from an accredited program.

• Current active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Illinois.
