Key Responsibilities\n

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues, playing an active role in the interdisciplinary care team.

• Conduct comprehensive patient intakes, performing thorough diagnostic and functional assessments to inform treatment recommendations.

• Deliver brief, focused therapeutic interventions to clients, catering to their unique mental health needs.
\n
• Advise primary care teams on referral processes for patients requiring specialized mental health care outside of the primary care setting.
\n
• Initiate and maintain follow-up contacts with clients to assess their progress and adapt treatment plans as necessary.
\n
• Develop relapse prevention strategies and assist patients in achieving stable mental and emotional functioning.
\n
• Identify “at risk” patients and create actionable plans to mitigate the escalation of their conditions.
\n
• Monitor the coordination of behavioral health services, ensuring that patients receive comprehensive care through effective communication with other treatment providers.
\n
• Maintain thorough documentation of patient diagnoses, progress, and treatment interventions in compliance with agency policies.
\n
• Work in partnership with primary care providers to formulate and implement effective treatment plans that address physical and mental health outcomes.

• Facilitate patient self-management by providing educational resources and supportive strategies to enhance their engagement in the treatment process.

• Contribute insights to clinic management regarding behavioral services, program evaluation, and community resource integration.

• Support patients in adhering to medical interventions recommended by primary care providers, including managing medication side effects.
\n
• Adhere to the Code of Ethical Conduct for Clinical Professionals and fulfill additional duties as assigned.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling is required.

• Current LPC (Licensed Professional Counselor) license in the state of Texas.

• Extensive knowledge in mental health assessment, treatment planning, DSM-IV diagnostic criteria, and therapeutic methodologies such as family and group therapy.

• Proficiency in both English and Spanish is highly desirable but not mandatory.

• Ability to assess individual patient needs effectively and develop targeted treatment plans to meet those needs.

• Strong communication skills to work collaboratively with interdisciplinary teams.

Compensation and Benefits\n

We offer a competitive salary range of USD 87,000 to $105,000 annually, commensurate with experience and qualifications. In addition to a generous salary, we provide a sign-on bonus for new hires.
