Key Responsibilities\n

• Acting as the primary liaison between families and outpatient providers to coordinate comprehensive care.

• Engaging with insurance companies for pre-certification processes and utilization management to ensure clients receive optimal services.

• Participating actively in discharge and aftercare planning to facilitate smooth transitions for clients leaving acute care settings.

• Providing therapeutic meal support with an emphasis on fostering a positive relationship with food and promoting holistic well-being.

• Delivering empathetic, safe, and affirming care tailored to individual clients, taking into account their unique backgrounds and intersecting identities.

• Documenting client progress comprehensively in compliance with state regulations and organizational standards.

Patient Population / Client Focus\n

This role primarily supports school-aged children, adolescents, and their families. Understanding the challenges faced by this demographic—such as academic pressures, socialization issues, and mental health challenges—is crucial. Qualifications\n

• Must hold a completed Master's degree in Social Work from an accredited institution.

• A valid Licensed Clinical Social Worker (LCSW) license in California is required.

Compensation and Benefits\n
