Key Responsibilities\n

  Engage in ongoing communication with physicians to report on patient progress, concerns, and interventions, ensuring a cohesive approach to care.

  Consult and collaborate with families or support systems regarding client treatment and effective discharge planning, fostering a supportive network for recovery.

  Maintain accurate and timely documentation, including clinical assessments and progress notes, in accordance with departmental policies, to track client progress effectively.

  Work closely with multidisciplinary healthcare teams to develop comprehensive treatment plans based on psychosocial assessments and evidence-based interventions.

  Assist clients throughout the discharge planning process, conducting care coordination to link them with regional services and resources needed for ongoing support.

Patient Population / Client Focus\n

Your primary focus will be on adult clients experiencing various mental health challenges, including anxiety, depression, trauma, and substance use disorders. By employing therapeutic best practices and individualized treatment plans, you will play an integral role in helping clients reclaim their lives and achieve their personal recovery goals. Engaging with clients through tailored therapy sessions, you will support them in navigating hurdles and maintaining stability in their daily lives.

Qualifications\n

  A Master’s degree in Social Work, Counseling, Marriage & Family Therapy, Psychology, or a related field.

  Current active state licensure as an LCSW, LMFT, LPC, or equivalent in Pennsylvania.

  A minimum of three years of experience providing outpatient mental health services.

  Proficient computer skills, including familiarity with electronic medical records, word processing, email systems, and spreadsheet software.

  Strong organizational skills and the ability to work independently, demonstrating self-direction and accountability.

Preferred Experience\n
