Key Responsibilities\n

The Licensed Professional Counselor will undertake a range of essential tasks to offer comprehensive care for our patients:

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues, playing an active role in the interdisciplinary team.

• Conduct thorough patient intakes that focus on diagnostic and functional evaluations, formulating personalized treatment goals in partnership with the primary care team.

• Provide brief, targeted interventions for clients seeking mental health services, ensuring evidence-based practices are applied for best outcomes.

• Advise on referrals, identifying patients who may benefit from specialized mental health facilities outside of the primary care environment.

• Initiate follow-up communications to assess treatment progress and recommend any necessary adjustments to care plans.

• Develop relapse prevention plans as needed, offering continuous support to maintain patients’ stability and minimize recurrences.

• Monitor the coordination of health services related to behavioral health care, actively linking patients with external treatment providers to enhance their support network.

• Maintain meticulous documentation of patient progress, assessments, and interventions within treatment charts to ensure compliance with agency and Federally Qualified Health Center guidelines.

• Provide consultation to clinic management and other team members about behavioral services, participating in outcome evaluations and program assessments as needed.

• Support patient adherence to any medical treatments prescribed by primary care providers, including coping strategies for medication side effects.

Qualifications\n

To be considered for this impactful role, candidates must fulfill the following criteria:

Compensation and Benefits\n

This full-time position offers a competitive salary range of USD 87,000 – 105,000 annually, commensurate with qualifications and experience. Additionally, employees are eligible for a sign-on bonus, enhancing the overall compensation package.
