Key Responsibilities\n

As a Licensed Professional Counselor at Mohawk Trail Counseling Center, your responsibilities will include:

• Conducting patient intakes to ensure accurate diagnostic and functional evaluations, enabling tailored treatment plans.

• Assisting primary care providers in identifying, treating, and managing mental health and psychosocial issues.
\n
• Providing brief, focused therapeutic interventions to patients requiring immediate mental health support.
\n
• Collaborating with primary care providers to determine the best treatment avenues, including referrals to specialty mental health facilities as needed.
\n
• Engaging in follow-up communications to gauge patient progress and adjust treatment plans accordingly.
\n
• Developing relapse prevention strategies and assisting patients in achieving stable functioning.
\n
• Identifying at-risk patients and designing plans to mitigate deterioration of their conditions.
\n
• Documenting patient progress and maintaining comprehensive treatment charts in alignment with Agency and Federally Qualified Health Center (FQHC) guidelines.

• Providing self-management skills and education to empower patients in their treatment journeys.
\n
• Consulting with management and team members on behavioral services and program effectiveness evaluations.
\n
• Ensuring adherence to the Code of Ethical Conduct for Clinical Professionals.

Patient Population / Client Focus\n

Your role will primarily involve working with individuals from diverse backgrounds, addressing various mental health issues. This can include children, adolescents, and adults dealing with anxiety, depression, trauma, and other behavioral health concerns. By fostering a supportive and trusting environment, you will encourage patients to actively participate in their treatment and overall recovery journey.

Qualifications\n

To excel in this role, candidates must possess:

• A minimum of a Master’s Degree in Clinical Counseling.

• A current LPC (Licensed Professional Counselor) license to practice.

• Thorough knowledge of DSM-IV diagnoses and behavioral therapy methodologies.

• Strong interpersonal skills, enabling effective relationship-building with patients, their families, and interdisciplinary teams.

Compensation and Benefits\n

The salary for this position ranges from USD 87,000 to 105,000 annually, with the actual salary being contingent on experience and qualifications.
