Key Responsibilities\n

• Conduct comprehensive patient intakes, focusing on diagnostic assessments and functional evaluations to tailor individualized treatment plans.

• Provide immediate intervention for patients requiring mental health services, ensuring timely access to care and support.

• Collaborate closely with primary care providers to recognize and manage mental health and psychosocial issues as part of a cohesive healthcare team.

• Develop and implement relapse prevention plans and assist patients in maintaining stable psychological functioning.

• Monitor and coordinate behavioral health services, liaising with external providers as needed to ensure comprehensive care.

• Document patient progress, update treatment charts accurately, and keep the primary care team informed of each patients’ evolving needs.

• Equip patients with self-management skills and educational resources that empower them in their journey towards recovery.

• Participate in team meetings and provide consultations on behavioral services, aiming for continuous improvement of patient outcomes.

Patient Population / Client Focus\n

You will serve a diverse population, addressing various mental health concerns and psychosocial challenges in a community setting. Our patients range from children to adults and present with a variety of needs, from anxiety and depression to complex psychosocial issues. Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling.

• Current LPC license for practice in the state of Pennsylvania.

Compensation and Benefits\n

The salary for this position ranges from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
