Key Responsibilities\n

• Engage in regular communication with physicians and healthcare team members to report on patient progress, needs, and concerns, ensuring that all treatment plans are comprehensive and current.

• Maintain accurate, timely clinical documentation and progress notes in alignment with department policies and state regulations, ensuring high standards of record-keeping.

• Collaboratively develop treatment and discharge plans that are informed by thorough psychosocial assessments and evidence-based practices, tailoring interventions to meet the unique needs of each client.

• Provide counseling and crisis intervention, assessing emotional and behavioral issues and employing appropriate strategies to promote mental health recovery.

• Assist clients with discharge planning and navigate care coordination with local providers, helping them to facilitate access to necessary services and resources.

• Engage in consultations with families and support systems to ensure comprehensive involvement in the treatment and discharge planning processes, fostering patient engagement and community support.

Qualifications\n
