Key Responsibilities

• Evaluate and assess patients in their home environment, analyzing their needs and establishing appropriate treatment goals.
• Implement individualized treatment plans that may include exercise regimens, manual therapy, and use of assistive modalities to diminish pain and enhance mobility.
• Monitor patient progress meticulously, adjusting therapy strategies as needed based on ongoing evaluations.
• Educate patients and their families about treatment protocols, the use of assistive devices, and home exercises to aid recovery.
• Maintain comprehensive and accurate patient records, reflecting treatment modalities used and the outcomes achieved to ensure continuity of care.
• Collaborate with multidisciplinary teams to provide holistic care and refine treatment plans based on patient feedback and clinical findings.
• Stay up-to-date with the latest advancements in physical therapy methods, techniques, and research through professional development opportunities.

Patient Population / Client Focus

Your primary focus will be on patients recovering from various physical conditions, including post-operative rehabilitation, sports injuries, chronic pain, and mobility issues. You will work with a diverse demographic—from children to seniors—each with unique needs and recovery goals. The personalized nature of home care allows you to build strong therapeutic relationships, adapting your approach to fit each patient's lifestyle and preferences.

Qualifications

• Doctorate degree in Physical Therapy (DPT) from an accredited program.
• Current and valid Florida state licensure to practice Physical Therapy.
• Excellent verbal and written communication skills, enabling effective interaction with patients, families, and healthcare team members.
• Proficiency in electronic medical record systems and basic computer applications for documentation and communication.
• A strong commitment to patient-centered care and a passion for helping others achieve their best health.

Preferred Experience

• At least 2 years of experience in a clinical setting, preferably with home care experience.
• Demonstrated agility and adaptability to manage varied patient scenarios and environments.
• Previous mentorship or leadership experience is a plus but not required.

Compensation and Benefits
