The ideal candidate will be instrumental in developing treatment plans and will ensure adherence to Agency and Federally Qualified Health Center (FQHC) compliance, all while meeting prescribed weekly billable service productivity targets. The salary for this position ranges from USD 90,000 to 105,000 annually.

While your primary responsibilities will be conducted remotely, you will collaborate closely with a multidisciplinary team that includes primary care practitioners and other specialty providers to create a holistic treatment plan for our patients. Key Responsibilities\n

• Collaborate with primary care practitioners to identify, treat, and manage mental health and psychosocial issues, thereby enhancing the overall effectiveness of the primary care team.

• Conduct comprehensive patient intakes focusing on diagnostic and functional evaluations, subsequently developing tailored therapy objectives and treatment recommendations.

• Provide immediate mental health interventions to clients requiring emergency support, ensuring timely access to care.
\n
• Guide primary care practitioners in determining which patients may benefit from specialized mental health services or necessary referrals to additional experts.
\n
• Perform regular follow-up evaluations to assess patients' progress and make necessary adjustments to treatment plans based on their evolving needs.

• Assist patients in formulating relapse prevention strategies and support them throughout their recovery journey.
\n
• Identify patients at risk for deteriorating health and collaborate on preventative programs to meet their unique needs.
\n
• Coordinate the delivery of mental health services, integrating care across various treatment modalities and collaborating with external providers as appropriate.

• Maintain thorough and accurate patient documentation, including progress notes and diagnostic information, ensuring current records are kept up-to-date.

• Communicate effectively with primary care providers to keep them informed about patients’ progress and actively participate in collaborative treatment planning.

• Encourage positive behavioral changes in patients at risk of physical and mental health issues, providing education on lifestyle modifications.

• Equip patients with essential self-management skills and resources to empower them in their recovery process.

• Consult with clinic management and team members on behavioral services, identifying areas for outcome evaluation and potential program development.

• Support patients in adhering to recommended medical therapies initiated by primary care practitioners and provide guidance on managing side effects of medications.

• Uphold the Clinical Professionals' Code of Ethics and carry out additional responsibilities as assigned.

Qualifications\n

• A Master's Degree in Clinical Counseling is required.

• A valid Licensed Professional Counselor (LPC) license is necessary.

• Bilingual fluency in English and Spanish is encouraged to better serve our diverse clientele.
