Key Responsibilities\n

As a Mental Health Counselor at Empower Care, your primary responsibilities will include:

• Conducting comprehensive psychosocial assessments for individuals, adolescents, and families to identify their needs and triage care appropriately.

• Implementing graduate-level clinical social work and counseling interventions that are aligned with best practices and client goals.

• Participating in patient rounds and interdisciplinary team meetings to communicate assessment findings and coordinate care efforts.

• Delivering case management services, collaborating with community professionals and programs to ensure comprehensive care for assigned patients.

• Performing timely and ethical documentation in accordance with department standards, enhancing communication and continuity of care.

• Engaging in Cognitive Behavioral Therapy (CBT) interventions and other evidence-based modalities to assist patients in their individualized service plans.

Qualifications\n

The ideal candidate will possess:

• A current and valid license as a Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), or Doctor of Psychology (PsyD).

• A Bachelor’s degree in Social Work, Psychology, or a related field, with a minimum of two years of clinical experience in mental health settings.
