Concentra PT and Wellness Clinic in Fort Lee, New Jersey is actively seeking a dedicated and skilled Licensed Physical Therapist to join our onsite team in a full-time capacity. In this role, you will play a vital part in enhancing the quality of life for our residents at our Assisted Living Facility, providing them with specialized physical therapy to maintain their mobility and independence.

Work Environment

Our Rehabilitation Center features a serene, residential setting that promotes peaceful interaction and recovery for older adults. The facility operates primarily during weekdays, allowing our therapists to maintain predictable schedules conducive to both staff and resident needs. The physical therapist's role is carried out onsite, providing direct care within apartments or designated therapy areas, ensuring minimal travel and maximizing patient engagement.

Key Responsibilities

• Conduct detailed physical therapy evaluations for newly admitted residents and ongoing assessments for current residents, ensuring personalized care strategies.
• Develop individualized therapy plans that align with each resident's personal goals, preferences, and functional needs.
• Implement one-on-one therapy sessions that may take place in residents' own apartments or in our specialized therapy spaces, facilitating comfort and familiarity during treatment.
• Monitor patient progress over time, adjusting treatment plans based on observed outcomes and documented data within our EMR system.
• Collaborate closely with nursing staff and wellness coordinators to design and refine safety, exercise, and fall prevention programs tailored to individual residents.
• Educate residents and their families on effective mobility strategies, the use of assistive devices, and recommended home exercise programs to encourage continued improvement outside of therapy sessions.

Patient Population / Client Focus

This role focuses primarily on older adults residing within our Assisted Living Facility. Our residents often face challenges related to mobility, strength, and balance, necessitating thoughtful, empathetic, and customized rehabilitation strategies to enhance their functionality and autonomy. By fostering a trusting relationship, the physical therapist contributes significantly to each resident's physical and emotional well-being.

Qualifications

• Active New Jersey Physical Therapist License is required.
• Graduate of an accredited Physical Therapy program (DPT, MPT, or equivalent).
• Current CPR/BLS certification.
• Effective interpersonal and communication skills necessary for supporting older adults and collaborating with a multidisciplinary team.
• A compassionate approach proven through experience with geriatric populations.

Preferred Experience

• Minimum of 2 years of professional experience in physical therapy, ideally within assisted living or rehabilitation settings.
• Experience in developing and implementing therapy programs focusing specifically on fall prevention and mobility enhancement for seniors.

Compensation and Benefits
