Key Responsibilities\n

• Conduct continuous assessments to evaluate and adapt treatment plans for clients, ensuring alignment with their evolving needs.

• Communicate effectively with supervisors, direct care team members, family, and other stakeholders to provide updates on patient progress and treatment outcomes.

• Present a structured schedule of appointments, home visits, and progress observations to facilitate seamless service delivery.

• Oversee the planning and coordination of home visits, enabling thorough assessments that complement clinic-based services.

• Ensure the effective implementation of individualized program plans through regular monitoring and adjustments based on client feedback.

• Participate actively in interdisciplinary team meetings, contributing insights that enhance collaborative care approaches.
