Work Environment

This role is predominantly onsite, as you will be visiting patients in their homes, creating an intimate and focused care setting. Key Responsibilities

• Conduct comprehensive evaluations and assessments of patients' physical abilities and limitations to establish effective treatment goals.
• Develop and implement individualized treatment plans grounded in evidence-based practices, collaborating with physicians, caregivers, and other healthcare professionals.
• Administer varied therapeutic interventions, utilizing techniques specific to orthopedic, pediatric, neurologic, and sports medicine fields.
• Provide patient education to enable clients and their caregivers to understand their treatment plan, promote wellness, and encourage adherence to home exercise programs.
• Document patient progress and outcomes meticulously, ensuring compliance with healthcare regulations and organizational protocols.
• Work collaboratively with other team members to optimize patient care, participate in case discussions, and contribute to the broader effectiveness of our service delivery.
• Strive to meet established utilization goals and other business objectives to support organizational success.

Patient Population / Client Focus

As a Full-Time Home Care Physical Therapist, you will engage with a diverse array of patients. Our clientele may include those recovering from surgeries, managing chronic conditions, or seeking rehabilitation after sports injuries. Your expertise will be vital in not only addressing their current challenges but also enhancing their long-term quality of life through tailored therapeutic interventions.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program recognized by the APTA or CAPTE.
• A valid and current Physical Therapy license in the state of Virginia.
• Basic Life Support (BLS) Certification from the American Heart Association.
• Effective verbal and written communication skills for ensuring clarity with patients, families, and a broader interdisciplinary team.
• Proficiency in word processing and basic computer applications to document treatment and track patient progress efficiently.
• Ability to build strong relationships with patients, enabling them to achieve and exceed their functional goals throughout their therapy.

Preferred Experience

• Minimum of two years' experience in a physical therapy setting, with a preference for home care or outpatient environments.
• Experience working with a variety of patient demographics, particularly in orthopedic and neurologic rehabilitation.
• Exposure to evidence-based practice initiatives, patient education, and community engagement activities.

Compensation and Benefits
