Key Responsibilities

• Conduct thorough evaluations to assess patients' physical conditions and develop customized treatment plans based on physician referrals and individual patient needs.
• Implement and execute evidence-based treatment protocols, demonstrating a wide range of techniques and procedures designed to enhance patient mobility and functionality.
• Provide skilled therapeutic interventions in patients' homes, ensuring that your care aligns with established physician plans while effectively addressing any diverse challenges that arise in the home care context.
• Engage actively with interdisciplinary teams to maximize patient outcomes through shared knowledge and collaborative strategies, placing patient needs at the forefront of all actions.
• Educate patients and their caregivers about injury prevention, home exercises, and general wellness, empowering them to take charge of their health.
• Uphold the highest standards of customer service, contributing positively to patient experiences and overall service effectiveness.
• Meet organizational goals and utilization targets while documenting outcomes and patient progress meticulously.

Patient Population / Client Focus

You will encounter a diverse patient demographic, including individuals with orthopedic conditions, neurological disorders, pediatric needs, and sports injuries. Your ability to adapt treatment modalities to each patient's unique situation is crucial in fostering optimal recovery.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited program is required.
• Current state licensure as a Doctor of Physical Therapy (DPT) through the Physical Therapy Examining Committee.
• At least two (2) years of direct patient care experience in a relevant clinical setting.
• Current CPR Certification to ensure patient safety and preparedness.
• Strong written and verbal communication skills to facilitate effective interactions with patients and healthcare teams.
• Proficient in using computer systems for documentation, scheduling, and communication.

Preferred Experience

• Experience in home health care settings is a plus, giving you an edge in familiarizing yourself with home-based therapy dynamics.
• A background in using telehealth technologies to enhance patient engagement and access to care.
• Proven ability to build strong relationships within the community and foster a collaborative network for comprehensive patient support.

Compensation and Benefits
