Key Responsibilities

• Conduct comprehensive evaluations for patients, including reviewing medical histories, performing standardized assessments, and determining individualized treatment recommendations based on assessment results.
• Communicate evaluation findings and treatment plans effectively to patients and their families, engaging them in goal-setting and ensuring their full understanding of the plan of care.
• Implement treatment protocols that are adapted to patients' specific conditions and the latest advancements in healthcare practices, applying evidence-based techniques.
• Consult and coordinate care with other healthcare professionals, documenting patient progress and necessary adjustments to the treatment plan based on evolving health needs.
• Complete discharge planning and ensure appropriate follow-up care, with meticulous documentation that outlines patients' journeys, including evaluations, treatment notes, and progress summaries.
• Demonstrate exceptional time management skills by meeting productivity expectations and maintaining an organized schedule, while also being punctual for meetings.
• Participate in community outreach and marketing initiatives to raise awareness about the clinic's services and our commitment to quality care.
• Contribute to departmental goals by actively engaging in quality improvement practices and ensuring compliance with regulatory standards.

Compensation and Benefits
