Work Environment

This role is performed onsite in a home care setting, where you will have the unique opportunity to treat patients in the comfort of their homes. This environment allows for personalized assessments and treatments tailored to each individual's specific needs and living conditions. The typical schedule for this position is Monday through Friday, from 8:00 am to 5:00 pm, providing a balanced work-life schedule.

Key Responsibilities

• Conduct comprehensive evaluations to assess patients' functional limitations, establishing a clear baseline for developing individualized treatment plans.

• Implement evidence-based treatment protocols that are responsive to each patient's unique presentation, ensuring that care is adapted to ongoing health changes.

• Collaborate with a diverse team of healthcare professionals, actively participating in interdisciplinary case discussions to optimize patient care and outcomes.

• Maintain accurate and timely documentation of patient care activities, from initial evaluations and treatment plans to discharge summaries, in compliance with healthcare regulations.

• Oversee discharge planning and follow-up care, ensuring continuity in patient management and support after therapy has concluded.

• Adhere to state practice acts and regulations, supervising physical therapy aides and ensuring compliance with all corporate policies related to clinical services.

• Engage in professional development opportunities and attend required training sessions to stay current on best practices in orthopedic physical therapy.

Patient Population / Client Focus

The ideal candidate will work predominantly with orthopedic patients recovering from surgeries, injuries, and those managing chronic conditions. The patient demographics are diverse, encompassing a wide range of ages and activity levels, necessitating adaptability and innovative treatment approaches tailored to each individual's rehabilitation journey.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy program is required.

• A valid physical therapy state license in Florida.

• Minimum of two years of clinical experience in orthopedic physical therapy, showcasing a strong foundation in patient assessment and therapeutic interventions.

• A current CPR certification.

• Excellent communication and interpersonal skills, enabling effective patient engagement and rapport building.

Preferred Experience
