Work Environment

This role requires you to provide care onsite, primarily in the home care setting, where you'll have the opportunity to interact directly with patients in a familiar environment. The work schedule is Monday through Friday, allowing for 40 hours a week with flexible time arrangements. Key Responsibilities

• Deliver high-quality, safe, and compassionate patient care, focusing on both acute and outpatient settings as needed.

• Implement and manage treatment plans for your patients, adhering to the physician-determined goals while adapting as necessary based on real-time evaluations.

• Conduct thorough assessments, including reviewing pertinent patient history and performing specific evaluations to determine the most effective treatment strategies.

• Document patient interactions and treatment outcomes clearly and accurately to maintain compliance with healthcare regulations, ensuring transparency in patient care documentation.

• Engage in meticulous discharge planning and ensure all necessary follow-up care is scheduled, contributing to a seamless transition for your patients.

• Participate in corporate and clinical training, maintaining compliance with all relevant local, state, and federal regulations.

• Collaborate with an interdisciplinary team of healthcare professionals, which includes fellow therapists and physicians, to create a well-rounded treatment approach for each patient.

• Attend facility meetings and participate in professional development opportunities as directed by your manager.

Patient Population / Client Focus

Your primary focus will be on orthopedic patients, including post-surgical rehabilitation and chronic pain management. You will leverage evidence-based practices to tailor treatment plans aimed at maximizing recovery and improving mobility for each individual. The ability to build rapport and understand diverse patient backgrounds will be essential in this role.

Qualifications

• Must be a graduate of an accredited Doctor of Physical Therapy (DPT) program.

• Possess an active physical therapy license as required by the State of Virginia.

• Current American Heart Association Basic Life Support (BLS) certification for Healthcare Providers is mandatory.

Preferred Experience

• A minimum of 2 years of professional experience in physical therapy is preferred, with a focus on orthopedic rehabilitation.

• Advanced manual therapy skills and familiarity with various therapeutic modalities will be assets.

Compensation and Benefits
