Key Responsibilities

As a DPT at Transformative Therapy Center, you will be responsible for the following:

• Delivering high-quality, safe, and patient-centered care in an outpatient setting, systematically adhering to treatment protocols.
• Implementing treatment programs based on established patient-centered care plans, adjusting as necessary according to patient progress.
• Conducting thorough evaluations involving the review of medical histories, detailed assessments, and generating comprehensive recommendations.
• Documenting patient interactions and maintaining up-to-date records including initial evaluations, daily progress notes, and discharge summaries to capture the patient's care trajectory and outcomes.
• Engaging in effective discharge planning and ensuring accessible follow-up care to empower patients towards self-management of their conditions.
• Communicating transparently and respectfully with patients, caregivers, co-workers, and third-party payors to foster cooperation and collaboration.

Patient Population / Client Focus

Qualifications
