Key Responsibilities\n

• Ensure adherence to program implementation guidelines, actively participating in the evaluation of treatment efficacy.

• Engage in interdisciplinary team meetings to collaborate on patient care strategies and case management efforts.

• Document monthly and quarterly progress notes reflecting treatment advancements and areas requiring further attention.

• Facilitate training sessions for staff to enhance their competence in applying program protocols and maintaining accurate documentation.

• Carry out follow-up interviews and additional home visits to provide continuous support and adjustments to treatment as needed.

Qualifications\n
