Work Environment

This role is set within a home care facility, emphasizing direct, personalized patient interaction. As a member of our team, you will work in a supportive and collaborative atmosphere, engaging closely with patients in their own homes to provide unparalleled care. Our work schedule operates Monday through Friday from 9:00 AM to 5:00 PM, ensuring a balanced professional environment that promotes work-life harmony.

Key Responsibilities

• Perform thorough evaluations and assessments to identify patients' goals and individual requirements for rehabilitation.
• Design, implement, and adjust personalized care and treatment plans based on the latest evidence-based methods and the specific needs of each patient.
• Deliver effective interventions aimed at restoring patients' function, enhancing mobility, and improving overall well-being.
• Create fitness and wellness programs that encourage ongoing health maintenance beyond recovery.
• Regularly monitor patient progress, evaluate treatment outcomes, and adjust care plans to ensure optimal results.
• Provide education and guidance to patients and their families about physical health and wellness to support long-term success.
• Collaborate with interdisciplinary teams of healthcare professionals to ensure holistic patient care.
• Inspire patient motivation and engagement through personalized encouragement and support.

Patient Population / Client Focus

Your primary focus will be on patients recovering from orthopedic conditions, including but not limited to sports injuries, post-surgical rehabilitation, joint disorders, and chronic pain management. By employing targeted therapeutic approaches, you will work closely with patients of all ages, creating custom paths toward their recovery while fostering a supportive and empathetic environment.

Qualifications

• A Master's Degree or Doctorate in Physical Therapy from an accredited institution.
• Active state license to practice as a Physical Therapist in New Jersey.
• A minimum of 2 years of experience specifically in orthopedic rehabilitation.
• Strong background in patient care, demonstrating proficiency in assessments, treatment plans, and direct client interaction.
• Excellent communication skills, with the ability to build rapport and effectively interact with patients, their families, and fellow healthcare professionals.
• Strong interpersonal skills with a compassionate approach to managing diverse patient personalities.

Preferred Experience

• Experience employing a variety of rehabilitation techniques, such as manual therapy, therapeutic exercise, and functional training.
• Familiarity with the latest advancements in orthopedic physical therapy and a commitment to ongoing professional development.
• Skills in using electronic medical records systems for documentation and care coordination.

Compensation and Benefits

This full-time position offers a competitive annual salary ranging from $110,000 to $130,000, depending on experience, in addition to performance-based bonuses.
