Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify the unique mental health needs and concerns of patients and their families.

• Provide individual, family, and group therapy sessions utilizing tailored evidence-based treatment modalities appropriate for hospice care.

• Develop, implement, and regularly review individualized treatment plans that align with patient goals, ensuring that interventions are compassionate and targeted.

• Facilitate therapeutic interventions that address grief, loss, and anticipatory mourning, helping patients and families navigate their emotional journeys.

• Collaborate with a team of interdisciplinary professionals to coordinate care plans and address emerging issues in real time.

• Offer crisis intervention services, including risk assessments and safety planning for patients exhibiting acute distress.

• Ensure thorough documentation of patient interactions and treatment plans, adhering to all ethical, legal, and organizational standards for record-keeping.

• Lead support groups and psychoeducation classes focused on coping strategies and self-care for patients and caregivers.

Patient Population / Client Focus\n

In this role, you will work with a diverse clientele, primarily consisting of terminally ill patients and their families. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
