Key Responsibilities\n

• Conduct comprehensive intake and psychosocial assessments to understand clients' mental and emotional needs, documenting findings in accordance with relevant guidelines.

• Develop and present tailored treatment plans to the Clinical Drug Rehabilitation Program and Services (CDRP/S) team, incorporating feedback and recommendations to ensure the best possible care.

• Engage in clinical case conferences, contributing insights and collaborating with other professionals to create cohesive treatment strategies for clients.
\n
• Deliver educational lectures focused on chemical dependency to enhance community understanding and support while fostering awareness about addiction issues.
\n
• Facilitate individual and group counseling sessions, specifically targeting those struggling with chemical dependency, ensuring a supportive and therapeutic environment.
\n
• Provide therapy sessions addressing adolescents, significant others, families, and multigenerational issues, adapting strategies to meet diverse needs as assigned.

• Offer comprehensive case management services, facilitating resource identification and referrals to enhance the therapeutic journey of patients.

Patient Population / Client Focus\n

Your practice will primarily focus on supporting hospice patients and their families, who are navigating the emotional complexities associated with end-of-life care. Building trusting relationships and enabling effective communication between patients and their families is vital. The role requires not only an understanding of end-of-life issues but also the ability to provide tailored support that aligns with patients' and families' unique needs and circumstances.

Qualifications\n

• A Master’s degree in Social Work (MSW) or a related field is required.

• Licensure/Certification: The candidate must hold a current Licensed Independent Clinical Social Worker (LICSW) license in the state of Washington.

• Demonstrated experience with at least two (2) years of post-graduate supervised work in addiction medicine therapy is essential, with a focus on providing sensitive and effective support for patients.

Preferred Experience\n

While our minimum standard is two years, candidates with additional years of experience in clinical social work, particularly in hospice or palliative care settings, will be viewed favorably. Familiarity with utilizing electronic health records (EHR) and a commitment to continuous professional development are also advantageous.

Compensation and Benefits\n
