Key Responsibilities\n

• Conduct comprehensive assessments to evaluate the mental health needs of patients, developing individualized treatment plans that address their specific circumstances.

• Facilitate communication with supervisors, family members, and other stakeholders regarding each patient's progress, ensuring transparent and consistent updates.

• Create and manage a schedule of appointments, home visits, and follow-ups to monitor patient advancements and ensure continuity of care.

• Coordinate in-home visits for detailed observations and assessments, collaborating with families to gather essential information for treatment and evaluations.

• Implement and monitor program plans, ensuring adherence to established therapeutic methods and making adjustments based on ongoing evaluations.

• Participate in Interdisciplinary Team Meetings to share insights and strategies for patient care, fostering a collaborative approach to mental health treatment.

• Maintain accurate and up-to-date patient records, ensuring they are accessible for interdisciplinary reviews and audits.

• Act as a liaison with external mental health resources and community organizations, advocating for the needs and goals of patients.

• Organize and facilitate training sessions for staff to enhance the team's competency in implementing therapy plans and best practices.

• Conduct follow-up interviews and observations to reinforce support structures for individuals, adapting care strategies as needed.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Possession of an active and unrestricted Professional Counselor (LPC) license in the state of Oregon.

• A minimum of two years of clinical counseling experience, preferably within community settings or clinics.

• Strong understanding of legal and ethical standards governing mental health practice, ensuring adherence to best practices and regulations.

Compensation and Benefits\n

The starting salary for this position ranges from $90,000 to $100,000 annually, contingent upon experience and availability of caseload.
