Key Responsibilities

• Evaluate patients in their home environment to understand their specific rehabilitation needs, considering individual circumstances such as living conditions and mobility challenges.
• Develop and implement personalized treatment plans that incorporate various modalities suited for home care, including therapeutic exercises, manual therapy techniques, and the use of specialized equipment.
• Utilize measurement tools to assess progress, document responses to treatment, and modify therapy as necessary to align with the evolving health status of each patient.
• Educate patients, caregivers, and family members on effective techniques for managing recovery and long-term care, including the proper use of assistive devices.
• Coordinate efforts with other healthcare professionals in a multidisciplinary approach to ensure integrated and effective patient care.

Qualifications

Compensation and Benefits
