Work Environment

This position is full-time and onsite, based in our state-of-the-art rehabilitation facility in Cape Coral, Florida. Key Responsibilities

• Conduct comprehensive evaluations of patients' physical abilities and limitations within a home care setting, specifically designed for transitional support as they recover from injuries, surgeries, or illnesses.
• Design and implement individualized treatment plans that incorporate a variety of therapeutic modalities such as therapeutic exercise, manual therapy, and patient education to alleviate pain and enhance mobility.
• Continuously assess patient progress and make necessary adjustments to treatment plans, ensuring that each therapeutic goal is achievable and aligned with the patient's recovery path.
• Document all patient interactions, treatment responses, and outcomes accurately in compliance with legal and organizational requirements, using our electronic medical record (EMR) system.
• Educate patients, caregivers, and family members on effective self-care practices and the proper use of assistive devices, ensuring understanding and compliance for optimal recovery.
• Participate in regular professional development through workshops and continued education opportunities to stay abreast of industry advancements and improve therapeutic techniques.

Patient Population / Client Focus

Our facility serves a diverse patient population, including children and adults with various diagnoses. You will work with individuals recovering from orthopedic injuries, neurological conditions, post-surgical rehabilitation, and chronic pain conditions. Your role will be pivotal in helping patients regain their independence and functional abilities, thereby improving their overall quality of life.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• A valid physical therapy license in the state of Florida.
• Strong verbal and written communication skills to effectively interact with patients, families, and team members.
• Proficiency in computer skills, particularly in word processing and using electronic medical records.
• A deep commitment to patient care and a passion for helping others achieve their health goals.

Preferred Experience

• While a minimum of 2 years of experience in a clinical setting is preferred, we encourage new graduates with a strong clinical rotation in relevant areas to apply.
• Experience with assistive devices and adaptive therapy techniques is a plus.

Compensation and Benefits
