Key Responsibilities\n

• Conduct ongoing client assessments to tailor treatment plans specific to patient needs, utilizing evidence-based practices.

• Facilitate ongoing communication with supervisors, direct care staff, family members, and external partners to ensure comprehensive treatment continuity.

• Create and share a tentative scheduling plan for appointments, home visits, and progress observations to monitor and evaluate treatment efficacy.

• Coordinate required home visits for thorough observations, including chart reviews and staff interviews, to gather insights for treatment plans.
\n
• Implement and monitor the effectiveness of strategy interventions in coordination with established program plans.
\n
• Participate actively in Interdisciplinary Team Meetings and any other relevant discussions to drive collaborative care.
\n
• Prepare detailed monthly and quarterly progress notes that reflect client status and treatment outcomes.
\n
• Design and organize training sessions for staff ensuring adherence to documented procedures and competency in implementing treatment plans.
\n
• Provide follow-up support through interviews and additional observations, ensuring clients' needs are consistently met.
\n
• Maintain precise and accessible records that document treatment progress, fostering transparency and support among interested parties.

• Act as a liaison with the public and mental health community to advocate for patient support and development initiatives.

• Contribute to the development of training materials aimed at enhancing program delivery for our clients.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related therapeutic field.
