Work Environment\n

This role is primarily remote, allowing for flexibility in work hours while interacting with patients through a secure digital platform. You will be working in an outpatient setting, conducting assessments, therapy sessions, and follow-ups with clients through telehealth visits. Key Responsibilities\n

• Conduct comprehensive assessments to evaluate the mental health needs of clients and tailor treatment plans accordingly.

• Regularly communicate with supervisors, health care teams, family members, and other stakeholders to discuss individual progress and therapeutic strategies.

• Develop and provide a tentative schedule of appointments, home visits, and therapy sessions while ensuring compliance with client needs.

• Coordinate home visits that may include chart reviews and staff interviews to gather insights and data crucial for effective treatment planning.

• Oversee the implementation of treatment plans, ensuring they are executed effectively to optimize patient outcomes.

• Participate actively in interdisciplinary team meetings to collaborate on patient care and program improvements while submitting progress notes on a monthly and quarterly basis.

• Organize training sessions for team members to ensure proficiency in the program implementation and adherence to documentation standards.
\n
• Maintain accurate and complete records related to patient care and outcomes, making information readily available to necessary parties.
\n
• Act as a liaison for the public and mental health community, promoting support for development goals and community mental health initiatives.

• Assist with creating and updating training materials aimed at improving care programs for diverse client populations.

Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Must possess active and unrestricted licensure as a Professional Counselor (LPC) within Arizona.

• Minimum of two years’ experience in counseling, ideally within a clinical context.

• Specialization in areas such as family therapy, couples therapy, substance misuse treatment, or trauma therapy is highly preferred.

• Demonstrable understanding of legal and ethical standards in mental health practice.

• Exceptional interpersonal skills with an emphasis on communication and rapport-building with clients from diverse backgrounds.

• A proven ability to work collaboratively within a multidisciplinary healthcare team.

• Familiarity with electronic medical record (EMR) software and other related technologies.
