Position Overview

Right Form Rehabilitation & Wellness Center in Avenel, New Jersey, is actively seeking a dedicated and compassionate physical therapist to join our team as a full-time staff member. Work Environment

As an onsite Physical Therapist at Right Form, you will work in an engaging and supportive rehabilitation center. Our facility is designed to offer a comfortable setting for both our staff and patients, promoting a healing environment. You will work autonomously while also collaborating with a multidisciplinary team comprising physicians, nurses, occupational therapists, and speech therapists, among others, to deliver holistic patient care. The work schedule is Monday to Friday, from 8:00 AM to 5:00 PM, providing a structured yet fulfilling work-life balance.

Key Responsibilities

• Conduct comprehensive evaluations of patients to assess functional limitations and disabilities, building individualized treatment plans based on these assessments.
• Remain informed about Illinois state practice acts and ensure compliance with all applicable laws governing physical therapy practices.
• Implement effective treatment protocols aligned with patient evaluations, needs, and the evolving health care landscape.
• Consult with other healthcare professionals regarding patient treatments and recognize when changes in a patient's condition require adjustments to the existing care plan.
• Maintain accurate documentation of patient interactions and care plans, ensuring a clear record of progress, interim assessments, and discharge summaries.
• Participate in discharge planning and follow-up care, focusing on the continuity of care for the patient.
• Engage in ongoing professional development to maintain current best practices and compliance with corporate requirements.
• Attending and participating in team conferences and meetings to discuss patient outcomes and improve service delivery.
• Actively contribute to initiatives aimed at removing barriers to patient access and improving service efficiency.

Patient Population / Client Focus

The patient population at Right Form encompasses a wide range of individuals struggling with mobility issues, post-operative rehabilitation, chronic pain, neurological conditions, and various musculoskeletal disorders. You will have the opportunity to work with diverse demographics, allowing you to apply and grow your skills while enhancing the quality of life for each patient you treat.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited institution.
• A valid physical therapist license in the state of New Jersey.
• At least two years of clinical experience in physical therapy, particularly working with varied patient populations.
• Current CPR certification to ensure safety and emergency preparedness.
• Exceptional interpersonal skills, with the capacity to build rapport with patients and engage effectively with colleagues.

Preferred Experience

• Experience in providing telehealth services or utilizing digital health applications to enhance patient care.
• Familiarity with interdisciplinary collaboration among diverse healthcare professionals.
• Background in mentoring or training junior staff or interns in the physical therapy field.

Compensation and Benefits

Right Form Rehabilitation & Wellness Center recognizes the hard work and dedication of our employees by offering a competitive salary range of $100,000 to $120,000 per year, complemented by an attractive benefits package.
