Position Overview

Right Form Rehabilitation & Wellness Center is actively seeking an experienced and dedicated Outpatient Physical Therapist to join our dynamic and passionate team in Newark, New Jersey. Work Environment

As an onsite physical therapist at our outpatient office, you will work Monday through Friday from 8:00 AM to 5:00 PM. Our facility is equipped with state-of-the-art rehabilitation equipment and resources that promote effective patient care. You will collaborate closely with fellow healthcare professionals, leveraging a multidiscipline approach to enhance treatment outcomes. Key Responsibilities

• Conduct thorough evaluations and assessments of patients with functional limitations and disabilities, developing effective individualized treatment plans based on their unique needs.
• Stay informed on relevant state practice acts and ensure compliance with regulations governing physical therapy practices.
• Implement treatment protocols with precision, adapting your approach based on patient progress and individual response to therapy.
• Communicate and consult with other healthcare providers as necessary, documenting any changes in patient conditions that may impact the ongoing treatment plan.
• Perform comprehensive discharge planning and follow-up, ensuring timely and accurate documentation of all patient care activities, including evaluations and progress notes.
• Engage in continuous learning by participating in team meetings, in-service training, and conferences to stay current on best practices in physical therapy.
• Contribute to initiatives aimed at removing barriers to healthcare access and enhancing patient engagement through collaborative projects.

Patient Population / Client Focus

The patients you will serve encompass a wide range of ages and backgrounds, often presenting with conditions such as post-surgical recovery, orthopedic injuries, neurological disorders, and chronic pain. Your expertise will be essential in assessing their capabilities and challenges, guiding them toward functional independence. Each patient interaction offers a chance to make a lasting impact on their health journey while fostering meaningful relationships built on trust and empathy.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited institution is required.
• Possession of a valid physical therapy license in the state of New Jersey.
• A minimum of 2 years of relevant experience in a physical therapy setting, with a focus on outpatient care.
• Current CPR certification is a must.
• Strong interpersonal skills with the ability to engage effectively with patients, fostering trust and rapport.

Preferred Experience

• Experience in personal training or wellness coaching, ideally with exposure to digital health solutions.
• Proficiency in evidence-based practice, utilizing outcome measures and assessments to enhance patient care.

Compensation and Benefits

This position offers a competitive salary ranging from USD 100,000 to 120,000 per year, plus a comprehensive benefits package designed to support your health and well-being.
