Work Environment

Located in the vibrant community of South River, NJ, our rehabilitation facility boasts modern treatment spaces equipped with the latest therapeutic tools and technologies. The onsite setting encourages active collaboration among healthcare professionals, ensuring that all team members work cohesively to deliver comprehensive care. The scheduled workdays are Monday through Friday from 8:00 AM to 5:00 PM, allowing for a balanced work-life experience.

Key Responsibilities

• Conduct thorough examinations and evaluations of patients with various physical limitations and disabilities, designing individualized treatment plans that address their unique needs.

• Stay informed about state practice acts and adhere to legal guidelines governing physical therapy practice and supervision standards.

• Provide detailed evaluation services, such as reviewing patient histories, performing specific assessments, and documenting both recommendations and results in a clear and timely manner.

• Expertly implement therapeutic treatment protocols based on evaluations, adapting to each patient's evolving health status and needs.

• Engage in interdisciplinary consultations, recognizing and documenting any significant changes in a patient's condition that may require modifications to their care plan or further medical consultations.

• Oversee discharge planning and follow-up care, ensuring that comprehensive records reflect the patient's journey through rehabilitation, including initial evaluations, ongoing progress notes, and discharge summaries.

• Maintain compliance with corporate and clinical training requirements while also adhering to local, state, and federal regulations.

• Effectively manage daily treatment schedules to guarantee quality care and maximize departmental productivity.

• Evaluate, treat, and direct therapy for patients requiring physical rehabilitation, documenting the medical necessity for their ongoing treatment.

• Participate actively in departmental meetings, conferences, and training sessions, promoting transparency and open communication across the team.

• Actively engage in initiatives to remove barriers to patient access and understand the relevant site measurement data as part of continuous improvement efforts.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy program is required.

• A valid state license to practice physical therapy in New Jersey is essential.

• Current certification in CPR is required to ensure the safety of all patients.

Preferred Experience

• At least two years of experience in a rehabilitation setting or a similar clinical environment is desirable. Familiarity with modern digital applications and solutions in physical therapy is a plus.
