Physical Therapist

Role Physical Therapist providing personalized, one‑on‑one care to older adults in their homes or senior living communities, utilizing a flexible schedule and virtual care options. Key Responsibilities Conduct evaluations, develop individualized treatment plans, and deliver evidence‑based therapy aimed at improving mobility, independence, and quality of life. Document patient progress accurately within electronic medical records, adhering to regulatory standards. Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists. Educate patients, caregivers, and staff on safety and aging-in-place strategies. Utilize telehealth and virtual tools to extend care access and flexibility. Qualifications & Skills Valid Physical Therapy license or eligibility in practice state(s). Degree from an accredited PT program. Proficiency in basic computer use and electronic medical records. Current CPR certification. Physical Requirements Ability to perform moderate physical effort, assist with patient transfers, reposition patients, stand/walk for extended periods, and drive between locations.
