Work Environment

This is an onsite position primarily oriented around home care services. You will be working directly in patients' residences, offering you a unique opportunity to provide customized treatment plans tailored to individual needs in a familiar and comfortable setting. Your work schedule will be Monday through Friday, encompassing a full-time commitment of 40 hours per week. Such an environment enables you to foster deeper connections with patients while contributing positively to their recovery processes.

Key Responsibilities

• Evaluate patients referred by their physicians to create personalized treatment programs that cater to their specific orthopedic needs.
• Implement and regularly adjust treatment interventions based on ongoing assessments and patient progress.
• Deliver skilled therapeutic care using modalities such as manual therapy, exercise prescriptions, and patient education techniques.
• Collaborate with a multidisciplinary team to share insights and strategies aimed at optimizing patient outcomes. This includes regular communication with referring physicians.
• Maintain detailed documentation of patient interactions, treatment plans, and progress notes aligned with regulatory standards.
• Conduct educational sessions for patients and their caregivers, ensuring they understand the rehabilitation process and can implement supportive measures at home.
• Meet or exceed established utilization goals and contribute to business objectives through effective patient engagement.

Patient Population / Client Focus

Your primary focus will be on orthopedic patients, but you will also have opportunities to work with pediatric, neurologic, and sports medicine populations. Understanding the diverse needs of these groups will allow you to apply targeted techniques designed for effective recovery. Your continuous engagement with patients will enable you to deliver a level of care that actively promotes movement, resilience, and overall health recovery.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program (APTA or CAPTE).
• A valid State License in Physical Therapy.
• Basic Life Support Certification from the American Heart Association.
• Excellent written and verbal communication skills.
• Proficiency in word processing and familiarity with electronic health records (EHR) software.
• Ability to establish strong therapeutic rapport with patients to help them achieve their functional goals.

Preferred Experience

• Previous experience in home care physical therapy, particularly in an orthopedic setting.
• Familiarity with evidence-based treatment methodologies and the ability to apply them in a home setting.
• Strong interpersonal skills with a genuine desire to create positive patient interactions.
• Ability to establish and maintain relationships with community organizations and stakeholders.

Compensation and Benefits

We offer a competitive salary range of USD 80,000 – 100,000 per year, commensurate with experience and qualifications.
