Key Responsibilities

• Conduct thorough evaluations of patients in their homes to assess their physical capabilities and limitations, designing tailored treatment modalities that align with their individual recovery goals.
• Develop and implement comprehensive treatment plans using a variety of therapeutic approaches, including therapeutic exercise, manual therapy techniques, and equipment utilization to alleviate pain and restore function.
• Engage patients in a collaborative process to establish personalized goals, providing motivation and support throughout their rehabilitation journey.
• Systematically track and document patient responses and progress through precise record-keeping, employing our advanced electronic health records system to ensure effective communication across our multidisciplinary team.
• Reevaluate patient progress and modify treatment strategies as needed, maintaining an adaptive approach to patient care.
• Collaborate with patients, families, and caregivers by offering education on assistive devices, self-care modalities, and effective therapy techniques to promote independence in daily living.
• Stay abreast of industry best practices and advancements in physical therapy, participating in professional development workshops and networking with peers.

Patient Population / Client Focus

Qualifications
