Key Responsibilities\n

• Develop and implement personalized treatment plans that include clear objectives, interventions, and measurable outcomes.

• Collaborate closely with physicians, nurses, and other healthcare professionals to ensure holistic and coordinated patient care.

• Facilitate group therapy sessions and psychoeducation workshops that empower patients and family members throughout the hospice process.

• Provide crisis intervention, risk assessments, and safety planning to manage urgent emotional and psychological challenges.

• Maintain detailed and accurate documentation of patient progress, therapy sessions, and treatment plans in compliance with ethical and legal standards.

• Assist with care transitions, ensuring emotional support is available as patients move through different stages of hospice care.

You will primarily serve patients facing terminal illnesses, along with their families, who may be experiencing a wide range of emotional responses such as grief, anxiety, and depression. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
